POLICIES

We believe that mental health treatment is one of the most important investments you can make for yourself and your loved ones. With an effort toward optimal treatment success please review following guidelines:

  • Mind & Matter Psychiatry is a private pay practice and does not currently accept insurance. Patients are required to pay at the time of service. To simplify this process a credit card may be kept on file.

  • Missed Appointments/Late Cancelations(cancelations with less than 24 hours of notice prior to scheduled appointment)

  • If you need to cancel an appointment for any reason, please provide at least 24 hours notice, otherwise your cancellation will be considered “late” any may include a cancellation fee based on our three-strike policy over a 12 month period. First late cancels/missed appointments will be waived as a courtesy. Second late cancels/no shows will be half of the cancellation fee. Third and following late cancels/no shows will be the full cancellation fee. We are defining a “cancellation fee” as the full cost of your scheduled visit. Cancellations of any appointment type will be applied towards the three-strikes policy. Additionally, after a third strike, the patient may be discharged from the practice.

  • Patients will be provided with a super-bill including the billing CPT codes appropriate for the completed appointment. These may be submitted to your PPO health insurance company. We unfortunately do not currently have the staffing to assist in this process further.

  • Please see the Fees section for specifics about cost of services unless a different rate has been discussed privately with your clinician. Fees upon entry are fixed for one year. After that, fees may be raised no more than annually.

  • Minimum frequency of appointments will be determined by your clinician as part of your treatment plan. In order to remain an active patient of your clinician, it is essential to consistently meet at the expected frequency. The minimum frequency of appointments typically ranges from every 1-3 months but may be higher depending on safety and medical standards of care.
